- D: Discuss and document. Discuss with Dawn the nature of the rehabilitation, prevention of angina, and ways to prevent pulmonary fibrosis exacerbation. Advise her to monitor symptoms of an angina attack and progression of fibrosis. Advise her to set up an appointment with her family doctor if possible to ensure that her angina and fibrosis are being managed. Additionally, discuss setting up respiratory therapy follow-up with the interdisciplinary team. Document her current mobility, the status of her short and long-term goals, and her treatment up to date.
- E: Educate the patient on their condition, on the home exercise program, next steps, symptoms of angina, fibrosis progression, the importance of follow-ups with their family doctor and respiratory therapist, and the importance of continuing physiotherapy.
- A: Assess Dawn’s level of understanding of the condition, outcome measures, and goals by getting him to dictate back to you what you have previously gone over. This also allows the patient to ask questions and leads to the last part of the discharge.
