After conducting your objective assessment, Paul presents with a myotome grade of 2 in L4, L5 on the right side and positive for a SLR at 60 degrees on the right. His reflexes on the left are +2 globally and on the right are +1. Babinski is negative bilaterally. His Quadratus lumborum is tight on the right when compared to the left, and his gluteus medius and minimus on the right are weak when compared to his left side. His piriformis is tight on the right. His L4,L5 spinous process is more tender on the right when compared to the left. He is negative in all of his SIJ testing.
